Official Graduate Studies Transcripts can be ordered through the Graduate Studies Office by completion and submission of an Official Transcript Request Form Graduate Studies including payment. (Note: transcripts will not be processed or released unless payment has been received).
Requesting your Official Transcript
There are several options you can use to request your University of Waterloo Graduate Studies Transcript. If you were last registered or are currently enrolled as an undergraduate student (Bachelor's degree), please visit the Registrar's Office website for information on ordering official undergraduate transcripts.
If you are Alumni of the University of Waterloo, you can use these options to make your request.

If you prefer, you can order your official University of Waterloo Graduate Studies transcript person by visiting the Graduate Studies Office.

When requesting your transcript by fax or mail, you will need to complete the Official Transcript Request Form Graduate Studies. The payment portion of the form must be completed in order for the transcript to be issued. (Note: transcript orders cannot be requested by phone).

For all ordering options please allow three to five business days (depending on service demands) for processing plus mailing time through Canada Post.
A non-refundable fee of $10.00 for each copy is required to process your transcript request. (Note: any undergraduate work completed at the University of Waterloo will be included with your University of Waterloo Graduate Studies Transcript). Also include any additional charges to courier or fax your transcript request(s) to the destination specified by you.
Note: receipts are not issued for payments received. Overpayments are not refundable.

The following processing options are available to you at the time you are placing your transcript order:

Courier and facsimile costs are in addition to the cost of each transcript request. (Note: you will not be notified when a transcript has been sent).

If you would like a third party to pick-up your transcript in-person from the Graduate Studies Office, a letter of authorization must be completed by you to allow a third party to collect the transcript.

When the third party comes in the Graduate Studies Office with the letter of authorization, they will be asked for ID which will be confirmed against the letter of authorization. Your signature will then be verified with what is on file before the transcript is released. Further, unless otherwise stated, the letter of authorization is good for one transaction only.
Note: Authorizing the release of a transcript does not authorize the third party to view or discuss a student's grades unless this is expressly stated within the letter.
If your current surname (family name) differs from your previous surname (your surname when you were last registered at the University of Waterloo) and you would like to have your current surname appear on your UW academic record and your official transcripts. Complete and attach a Change of Name form with the required supporting documentation to your transcript request.
